You'll want to enter in your Employee ID and your Personal Identification Number (PIN).
Your Employee ID number is a unique 6-digit number that SpaceX assigned to each employee upon hire. You can find your SpaceX Employee ID number by logging into SpaceX's HRIS system, Workday.

Your PIN is a combination of your 2 digit birth month and your 4 digit birth year in the following format: MMYYYY.
Example:
Birthdate = 05/22/1975
PIN = 051975
